Spinal injuries - osteoporosis fractures are one form of this

Anyone who injures their spine in an accident or fall usually hopes that they have not broken anything. However, there are a wide variety of spinal fractures - many of which do not require surgery to heal properly. Osteoporosis patients (and occasionally patients with osteoporosis) who suffer from brittle bones also often injure their spine, sometimes just by lifting or standing up. The best way to treat your spinal injury is to put yourself in the hands of our specialised surgeons right from the start. In this way, you can rule out any deterioration that could lead to long-term deformities.

Spinal injuries can occur at any age, depending on the mechanism of the accident.

When a vertebral body or joint breaks

The fracture of a vertebral body or parts of the vertebral joints can lead to pain, misalignment and loss of function. Care is also taken to ensure that spinal fractures can be ruled out during examinations for whiplash injuries. In order to make treatment as efficient as possible, it is important that the experts recognise possible instabilities of the spine as quickly as possible.

Inside the spine is the spinal canal, where the spinal cord and its central nervous system pathways run. In the event of a spinal fracture, there is always a risk of neurological complications developing due to damage to these nerves. The spinal surgery team at Bethesda Hospital has extensive experience in recognising such complications at an early stage and providing targeted treatment.

A spinal fracture hurts

Anyone who breaks or fractures a vertebra feels severe pain: the level of suffering can be very high, especially after accidents, and should be alleviated as quickly as possible by a specialised examination. Older people who suffer from osteoporosis can also fracture their spine after a clumsy movement. In certain cases, the fractures can also manifest themselves as paralyses in the limbs.

Have you injured your spine?

Rapid imaging is very important in the event of pain or injury in the spinal region. This ensures that a potentially unstable injury after an accident is not overlooked. Standard procedures include X-rays, but above all computerised tomography (CT) and occasionally also MRI.

A whiplash injury is diagnosed on the basis of the accident and the characteristic symptoms. In order to rule out visible injuries to the cervical spine, X-rays or an MRI scan are usually carried out.

If there is a suspicion of underlying osteoporosis, a bone density measurement is recommended and, if necessary, appropriate treatment with medication.

At our clinic, this is easily possible thanks to our close interdisciplinary collaboration with the rheumatology department.

Consolidation of the vertebra with the help of cement

As the severity of a spinal fracture can vary greatly, the treatment also varies greatly from person to person. The majority of spinal fractures can be treated without surgery. Occasionally, the fitting of a supportive corset or lumbar brace can be useful. Pain medication is also helpful, especially in the early stages.

In the case of osteoporosis fractures, where a vertebra increasingly collapses and loses height due to low bone density, surgery is usually necessary. This involves stabilising and rebuilding the vertebral body by filling it with cement (vertebroplasty). If there is or threatens to be additional instability, stabilisation with screws or rods can also be helpful.

Risk of osteoporosis in older people

Spinal injuries can occur at any age, depending on the mechanism of the accident. As it is mainly older people who suffer from diminishing bone density, osteoporosis fractures occur mainly in the later stages of life.

Our senior surgeons have extensive experience in the treatment of all spinal injuries thanks to their many years of working at trauma centres. However, spinal injuries resulting from car or bicycle accidents or falls from a great height must be admitted to and clarified at the University Hospital Basel, as concomitant injuries (internal haemorrhages or extremity injuries) may be present that are not part of the treatment offered at Bethesda Hospital.

In the case of injuries that occurred some time ago and are not quite as serious, as well as osteoporosis fractures, we will be happy to provide you with an assessment during consultation hours. Of course, our emergency ward is also available to you at any time for such cases by referral from your general practitioner's consultation.

Osteoporosis fractures can usually be treated without surgery. The initial severe pain gradually subsides. However, there are also cases in which the vertebra continues to collapse and misalignments or instabilities develop over the course of weeks. It can also happen that the fracture does not heal properly (so-called pseudarthrosis).

If such cases are recognised at an early stage, a small, minimally invasive procedure is often sufficient to prevent further deterioration and effectively alleviate the pain. A simple cementing procedure, known as vertebroplasty, is then completely sufficient.

However, if you wait too long, this operation is no longer effective and major stabilisation is often necessary. It is therefore crucial that the non-surgical treatment of vertebral fractures is also accompanied or carried out by spinal surgery from the outset.
